The list of documents for the admission to Kosmos Regional Children’s Center for Medical Rehabilitation

An outpatient medical record and an extract from the outpatient medical record, specifying the main and concomitant diseases, previous diseases, allergy, previous treatment and its efficacy.
If a patient has a concomitant disease, a medical conclusion issued by a relevant medical specialist (endocrinologist, otolaryngologist, allergy specialist, neuropathologist, ophthalmologist, etc.) shall be provided.
ECG (for cardiac patients).
A dentist’s conclusion (sanation of the oral cavity).
The certificate of health and the certificate, stating that a patient did not contact with any infectious disease patients within three weeks.
The certificate stating that a patient has not any infectious skin diseases or pediculosis.
The results of the complete blood count and urinalysis.
The results of the stool test for parasite eggs.
A complete copy of an immunization record card.
For schoolchildren: a certificate from school and a list of current grades.
